OIG exclusion screening

Each staff record keeps a history of exclusion checks with the date and the list version. A confirmed match makes that person unschedulable — the database refuses the assignment rather than warning about it.

A potential match is not a confirmed one. It stays open until somebody adjudicates it and writes down what they concluded, because 'we checked and it was a different J. Smith' is the answer a surveyor wants and the one nobody remembers a year later.
